Cancel more than 48 hours before your appointment and you receive a gift card for the full amount paid, usable by anyone. Cancel within 48 hours and a $50 fee is deducted from the gift card. OneLife members are exempt from the $50 fee entirely.

Yes. All tiered screening packages — Signature, Silver, Gold, and Platinum ($186–$483) — are HSA/FSA eligible, allowing you to pay with pre-tax dollars and effectively reduce your net cost.

Both packages cost $249 and add gender-specific tests to the core cardiovascular screening. Women's Essentials adds Thyroid and Osteoporosis screenings; Men's Essentials adds Testosterone Level and Prostate Cancer screenings. These packages are not available in AL, MA, ME, NY, or NV.

Order a kit online, collect your sample at home, and mail it to the laboratory. Results are returned in writing. Life Line provides one free replacement kit if the lab cannot process your sample or if the kit arrives damaged. Register and return your kit before the expiration date on the box.
